How Our Sliding Scale Fee Works

Our clinic offers a Sliding Scale Fee (SSF) to help offset medical service fees for all patients. The percentage of the adjusted rate varies depending on several factors, including household income and special financial circumstances. Our discounted medical services are available for qualified patients who do not have health insurance, families choosing not to pay for services with insurance or those which would suffer a financial hardship from paying their insurance copay. This discount is referred to as our “Sliding Scale Fee” or “SSF”. Requests for discounted services may be made by patients, family members, social services staff or others who are aware of existing financial hardship. The SSF does not apply to any travel fees which apply to mobile services.

Those with incomes at or below 100% of the poverty level will receive a full 100% discount. Those with incomes above 100% of poverty, but at or below 200% of poverty, will be charged according to our sliding fee schedule. Incomes falling in the middle will be assessed a discount according to amount listed on the SSF chart. The sliding fee schedule will be updated during the first quarter of every calendar year with the latest Federal Poverty Guidelines.

How to Begin Services With Our Office:

***OUR OFFICE IS NOT CURRENTLY ACCEPTING NEW PATIENTS FOR INDIVIDUAL AND FAMILY SERVICES. PLEASE CONSIDER JOINING ONE OF OUR GROUPS INSTEAD.***

1. Schedule an initial 30-minute caregiver consultation by clicking here. Your consultation will include a brief discussion of your family's reason for seeking services, as well as a recommendation for treatment.

2. A referral will be completed or services will begin with our office. If receiving a referral, it will be emailed to you and a follow-up will take place within 7 days. If beginning with our office, a link to complete the intake forms (Consent Forms, Informed Consent, SSF Information, SSF Application) within 7 days of the initial consultation.

3. Within 14 days of submitting your completed documentation, you will receive an email containining your confirmation of services, your finalized reduced fee, as well as a link to schedule and pay for your services.

4. The initial appointments you schedule will consist of an assessment period.

5. After the assessment is complete, you will use the same link to schedule ongoing services. Once the treatment length has been reached, treatement will be evaulated and possibly extended based on your family's needs.
